Mmmm I do not think it derives from a Fiat 500. The 500 (”Cinquecento”) had the engine, transmission and wheel-drive in the back of the car. In addition, that looks like a “big” engine compared to the one fitted in the 500.
My suggestion is that this vehicle was used for demonstrations in driving schools and/or technical institutes. My dad attended a technical institute in Italy back in the 60’s and I remember him telling me about vehicles like this that were used also for examinations. Students were asked about the name and function of the different parts, that’s why there are so many cut-away pieces
